It's rare as in it doesn't happen for everyone all the time. In your case, the 0.145 binary is obviously giving you issues. That is why I suggested upgrading to a later U release to see if it crashes. Check the NEWS section for MAME 0.145u1-0.145u4 64-bit Binaries to try.
