The jumper labeled ALT/MAME switches between the default key assignments for MAME shown here...

Are you saying that you need a button mapped to the ALT key plus the R key at the same time (ALT+R)? I don't think it's possible to map key combinations.

Or do you mean the ALT key on the right side of the space bar? I'm pretty sure this is possible.
