A couple of things:
- A little more info plz... arcade monitor or lcd or crt or ?
-Does the motherboard have onboard video that you turned off in the bios 1st or is the video only the PCIe slot? Is the signal coming from the AVGA(I imagine it is?)
-Did you run the installer as admin?
-What does the failure report say?
-Did you try to Update Driver for the "Basic Graphic Controller" & then force install the Ultimarc ArcadeVGA Drivers off of the CD?
FWIW - I haven't tried installing this since I fought with mine with Windows 8 so these are just basic suggestions to try. Actually I didn't know that the drivers were updated for Win 10 until I saw your post & looked, but it sounds & looks like, from the new web page, that this process is a WHOLE lot easier now. Hopefully your problem is just a simple one & 1 of these will get you going in the right direction.
Another thing to watch out for is updates...Windows used to try to update the old AVGA drivers to the newest version of the ATI drivers available every time since they were outdated. This of course wiped out all the specialty drivers needed for the card to work correctly in MAME, & thus if you let an update slip by you it meant uninstalling everything & doing it again. I'm not sure how Windows 10 handles this install now, seeing as how it upgrades everything for you without asking permission unless you circumvent it.
