Turns out it was an issue with our simulation of how ProDOS writes out sectors. There's a small delay at one point that makes no difference the vast majority of the time, but this crack depends on it to load properly. So it wasn't a problem with our understanding of the hardware, but rather a problem with our understanding of the software. It works now. Big thanks to Peter Ferrie for finally figuring it out.
