Midnight Resistance ends after one life

So yeah, Midnight Resistance ends after one life despite the dip settings set to four. I CAN 'continue' but it kinda kills the experience.

I've deleted ini and cfg files and started afresh (I don't see an nvram folder for the game) but it still happens. Any ideas what's causing this?
